Celecoxib and its Uses

Generic Celecoxib is a non-steroidal anti-inflammatory drug (SAID) used in the treatment of osteoarthritis, rheumatoid arthritis, acute pain, painful menstruation and menstrual symptoms.

Generic Celecoxib was approved by the FDA in 1998.

Generic Celecoxib is a prescribed for:

  • Relief of the joint pain, swelling, and stiffness of osteoarthritis (OA). OA is a type of arthritis caused by wear and tear on bones and joints
  • Relief of the joint pain, swelling, and stiffness of adult rheumatoid arthritis (RA). RA is an autoimmune disease that attacks healthy joint tissue, starting a process of inflammation and joint damage
  • Management of acute pain in adults (such as pain from strains and sprains) or pain after surgery
  • Treatment of primary dysmenorrhea (painful menstrual cramps)
  • Relief of signs and symptoms of ankylosing spondylitis, a painful form of arthritis that primarily affects the spine. Ankylosing spondylitis usually strikes young adults. It can cause inflammation, pain and stiffness in the shoulders, knees, hips, ribs and feet

Working Mechanism of Celecoxib

Generic Celecoxib works by blocking the action of a substance in the body called cyclo-oxygenase. Cyclo-oxygenase is involved in producing prostaglandins, in response to injury or certain diseases. These prostaglandins cause pain, swelling and inflammation. Because NSAIDs block the production of these prostaglandins, they are effective at relieving pain and inflammation.

Cyclo-oxygenase does not only produce prostaglandins that cause inflammation. Generic Celecoxib also produces prostaglandins that have useful roles in the body. There are two different forms of cyclo-oxygenase, COX-1 and COX-2. COX-2 is the form that (among other things) produces prostaglandins that cause inflammation. COX-1 does not produce inflammatory prostaglandins, but does produce others that have useful effects, including some that are involved in maintaining a healthy stomach and intestinal lining.

Missed Dose of Celecoxib

Take the missed dose of Celecoxib as soon as you remember. If it is almost time for the next dose, skip the dose of Celecoxib you missed and take only the next regularly scheduled dose as directed. Do not take a double dose of Celecoxib unless your doctor directs otherwise.

Excess Dose of Celecoxib

Generic Celecoxib in excess dose may show symptoms such as drowsiness, nausea, vomiting, stomach pain, dizziness, headache, ringing in the ears, blurred vision, little or no urine production, slow breathing, and coma.

Conditions for Storage of Generic Celecoxib

Store Generic Celecoxib at room temperature away from moisture and heat.

Food and Drug Interactions with Celecoxib

  • There may be an increased risk of bleeding if Generic Celecoxib is taken by people taking anti-blood-clotting medicines (anticoagulants) such as Coumadin (Warfarin). If you are taking an anticoagulant you should have your blood clotting time (INR) monitored while taking Generic Celecoxib, particularly in the first few days after starting treatment or if your dose is changed.
  • Generic Celecoxib may be taken with low-dose (75mg daily) aspirin, but should not be taken in combination with higher doses of aspirin or other non-steroidal anti-inflammatory drugs (NSAIDs), e.g. ibuprofen, diclofenac, as this may carry an increased risk of side effects on the stomach or intestines.
  • Generic Celecoxib may potentially decrease the effectiveness of medicines used for lowering high blood pressure (antihypertensives), e.g. diuretics such as bendroflumethiazide.
  • There may be an increased risk of side effects on the kidney if Celecoxib is taken with any of the following medicines, and your kidney function should be monitored if you are taking any of these medicines with Generic Celecoxib:
    • ACE inhibitors, e.g. enalapril
    • ciclosporin
    • diuretics, e.g. bendroflumethiazide, furosemide
    • tacrolimus.
  • Generic Celecoxib may increase the blood level of lithium. Your lithium blood level should be monitored if you are starting, changing dose, or stopping Celecoxib while taking lithium.
  • Other NSAIDs can reduce the removal of the medicine methotrexate from the body. This has not been reported with Dose of Celecoxib, but people taking methotrexate should be well monitored for any side effects of methotrexate while taking Celecoxib.
  • There is a possibility that Generic Celecoxib may increase the blood levels of the following medicines:
    • dextromethorphan
    • antidepressants (tricyclics, e.g. amitriptyline and SSRIs, e.g.
    • antipsychotic medicines (e.g. haloperidol)
    • anti-arrhythmic medicines (e.g. flecainide).
  • Fluconazole may increase the blood level of Celecoxib and your dose of Celecoxib should be halved if you also take fluconazole.
  • There is a possibility that the following medicines may reduce the blood level of Celecoxib:
    • rifampicin
    • carbamazepine
    • barbiturates

Contraindications with Celecoxib

Generic Celecoxib is contraindicated in the following

  • Children
  • Allergy to medicines from the sulphonamide group, e.g. the antibiotic sulphamethoxazole
  • Active peptic ulcer
  • Bleeding from the stomach or intestines
  • Inflammatory bowel disease such as ulcerative colitis or Crohn's disease
  • Severe liver disease
  • Moderate to severely decreased kidney function
  • Moderate to severe heart failure
  • Heart disease caused by inadequate blood flow to the heart (ischaemic heart disease), e.g. angina or history of heart attack
  • Disease of the blood vessels in and around the brain (cerebrovascular disease), e.g. history of stroke
  • Pregnancy
  • Breastfeeding
  • Women who could get pregnant, unless effective contraception is used to prevent pregnancy
  • People who have developed asthma, nasal inflammation (rhinitis), nasal polyps, swelling of the face, lips and tongue (angioedema) or itchy rash (urticaria) following the use of aspirin or other NSAIDs
  • Rare hereditary problems of galactose intolerance, the Lapp lactose deficiency or glucose-galactose malabsorption
  • Allergy to one or any of its ingredients

Precautions with Celecoxib

Generic Celecoxib should be used with caution in

  • Elderly people
  • Moderately reduced liver function
  • Mild to moderately decreased kidney function
  • Decreased volume of fluid in the body, e.g. due to high doses of diuretics, severe vomiting or diarrhea
  • Swelling due to excess fluid retention (oedema)
  • History of diseases of the stomach or intestines
  • History of ulceration or bleeding in the stomach or intestines
  • High blood pressure (hypertension)
  • Raised levels of fats such as cholesterol in the blood (hyperlipidaemia)
  • Diabetes
  • Smokers
  • Narrowing of the arteries in the extremities (peripheral arterial disease)

The adverse effects associated with Generic Celecoxib are

  • Difficulty in sleeping (insomnia)
  • Inflammation of the sinuses (sinusitis)
  • Dizziness
