Nolvadex and its Uses

Generic Nolvadex (tamoxifen) works by blocking estrogen. In breast tissue, Generic Nolvadex is an antiestrogen. An antiestrogen or estrogen blocker works by blocking estrogen in breast tissue. While estrogen may not actually cause breast cancer, it may stimulate its growth, feeding the cancer. With estrogen blocked, the cancer cells that need it may not grow at all. In other words, antiestrogens may keep cancer from developing in your breast.

Generic Nolvadex is used to treat advanced breast cancer. Generic Nolvadex has been successfully helping save the lives of millions of women with breast cancer for over 20 years.

Generic Nolvadex is very comparable to Clomid, works in the same manner in all tissues, and is a mixed estrogen agonist/antagonist of the same type as Clomid. The two molecules are also very similar in structure. Nolvadex is also used to treat infertility.

Generic Nolvadex is not indicated for the treatment of pre-menopausal, node positive patients. Because it can delay or stop the growth of breast cancer cells, it is also a welcome option for the management of ductal carcinoma in situ (DCIS) following surgery and radiation. Nolvadex Mechanism Nolvadex Working

The way in which Generic Nolvadex works is quite complicated and not yet fully understood, but its main function is as an anti-oestrogen drug.

Most breast cancers need supplies of the female hormone oestrogen to grow. Cancer cells have proteins called receptors on their surface that the sex hormones attach to. Cancers with oestrogen-receptors on the surface of their cells are called 'oestrogen-receptor-positive' (ER-positive) and Generic Nolvadex is most effective against these cancers. This reduces the size of oestrogen-sensitive tumors.

Under normal circumstances, when oestrogen comes into contact with the receptors, it fits into them and activates the cancer cells to divide so that the tumor grows.

Generic Nolvadex imitates the action of oestrogen and fits into the receptor but does not activate the cells to divide. The Generic Nolvadex stays in place and stops oestrogen from reaching the cancer cells so that they either grow more slowly or stop growing altogether.

Generic Nolvadex can greatly reduce the chance of oestrogen-receptor-positive cancers coming back after surgery. Nolvadex medication can also be used to shrink large tumors down before surgery so that they can be removed.

Generic Nolvadex is also used in the treatment of female infertility that is caused by problems with ovulation. Again, Nolvadex acts by blocking oestrogen receptors, this time in a part of the brain called the hypothalamus. The effect of this is an increase in the levels of the hormones that control the development and release of an egg. These hormones are released from the pituitary gland and are known as follicle stimulating hormone (FSH) and luteinising hormone (LH). FSH stimulates the ovaries and LH causes the release of an egg from the ovaries (ovulation).

In the normal menstrual cycle, oestrogen acts on receptors in the hypothalamus after ovulation. This causes the pituitary gland to stop releasing FSH and LH. As Nolvadex blocks the oestrogen receptors in the hypothalamus, it prevents this action of oestrogen. FSH and LH levels therefore increase, which increases the chances of egg development and ovulation.

Missed Dose of nolvadex

Take the missed dose of nolvadex as soon as you remember. However, if it is almost time for the next dose, skip the missed dose of nolvadex and take only the next regularly scheduled dose. Do not take a double dose of Nolvadex unless your doctor directs otherwise.

Food and Drug interactions with Nolvadex

  • Nolvadex decreases the removal of the anticoagulant medicines nicoumalone and warfarin from the body and hence increases their anti-blood-clotting effect. Women taking anticoagulants in combination with Nolvadex should have their blood-clotting times regularly monitored, and your doctor may need to reduce the dose of your anticoagulant.
  • There may be an increased risk of abnormal blood clots in the veins (thromboembolism) if Nolvadex is used in combination with cytotoxic chemotherapy medicines. Your doctor may prescribe you a medicine to prevent blood clots if you are having chemotherapy with Nolvadex.
  • There is a possibility that the SSRI antidepressant paroxetine may decrease the effect of Nolvadex. This is by no means an established interaction and has not been definitely confirmed, however the possibility that paroxetine might make Nolvadex less effective should be taken into account before it is prescribed in combination with Nolvadex.

Contraindications with Nolvadex

Nolvadex is contraindicated in the following

  • Infertility patients with a personal or family history, or genetic risk, of abnormal blood clots in the veins with no known cause
  • Pregnancy: Do not take Nolvadex without first talking to your doctor if you are pregnant or could become pregnant during or for two months following treatment. Effective non-hormonal contraception (birth control) should be used during and for two months following treatment with tamoxifen.
  • Breastfeeding: Do not take Nolvadex without first talking to your doctor if you are breast-feeding a baby.

Adverse effects with Nolvadex

Adverse effects are more common in pre-menopausal women, who may develop menopausal side effects as a result of a lowered level of oestrogen. The most common side effects, apart from nausea, are hot flushes and sweats, particularly at night.
  • Flushes and sweats: Sometimes the flushes will gradually lessen over the first few months but some women continue to have them for as long as they take Nolvadex. There are a number of ways to help to reduce or control hot flushes and sweats. Some women find it helpful to avoid or cut down on tea, coffee, nicotine and alcohol.

Sometimes other drugs can be prescribed to reduce hot flushes and sweats. Recent research suggests that progestogen or some types of antidepressant drugs may be helpful in controlling this unpleasant adverse effect. Your nurse or doctor can discuss this with you.

Sometimes the hot flushes continue for a long time after you have stopped taking Nolvadex.

  • Nausea and indigestion: Feeling sick (nausea) and indigestion are fairly common but can often be relieved by taking your tablets with food or milk or at night. Although nausea is quite common initially, it usually wears off after a few weeks.
  • Weight gain: Some women notice that they put on weight while taking Nolvadex. This may be due to water retention.
  • Change in periods: Women who have not yet reached the menopause may notice that their monthly periods change - they may become irregular, lighter or sometimes stop altogether. Some women also notice an increase in vaginal discharge and itching of the vulva (the area around the vagina).

Less common adverse effects of Nolvadex include

  • Depression, tiredness and dizziness: Some women feel depressed while taking Nolvadex but this may be due to other causes.
  • Headaches: Some people affected by migraine have noticed a change in the pattern of their headaches.
  • Thrombosis (blood clots): Pain, warmth, swelling or tenderness in an arm or leg or any chest pain must be reported to your doctor immediately.
  • Visual problems: Blurred or reduced vision is very rare but any visual changes should be reported to your doctor.
  • Voice changes have been reported by some women. Professional singers may want to seek help and advice from their doctor.
  • Tumor flare: Rarely, women who are prescribed Nolvadex for advanced cancer, where it has spread to the bones, may have tumor flare when they start taking Nolvadex. Tumor flare can cause a raised level of calcium in the blood (hypercalcaemia) with symptoms of nausea, vomiting and thirst. Very occasionally, a short stay in hospital is necessary until the calcium levels have been reduced.

Very rare side effects of Nolvadex include: mild allergic reactions, which may include skin rashes, and temporary thinning of the hair.
