Vaseretic and its Uses
Generic Vaseretic contains two active ingredients, enalapril and hydrochlorothiazide. Vaseretic is used to treat high blood pressure.

Enalapril is in a class of medications called angiotensin-converting enzyme (ACE) inhibitors. Generic Vaseretic works by block the action of a chemical in the body called angiotensin converting enzyme (ACE). Normally, ACE produces another chemical, angiotensin. Thus enalapril reduces the amount of angiotensin in the blood.

Hydrochlorothiazide is in a class of medications called diuretics ('water pills'). It works by causing the kidneys to get rid of surplus water and salt from the body into the urine.

Vaseretic Mechanism Vaseretic Working
Angiotensin
has two actions. Firstly it acts on blood vessels to make them narrow and secondly it acts on the kidney to produce less urine.

As enalapril stops the production of angiotensin, these actions are reversed. Therefore the kidneys produce more urine, which results in less fluid in the blood vessels. The blood vessels also widen. The overall effect of this is a drop in blood pressure and a decrease in the workload of the heart.

Hydrochlorothiazide is known as a thiazide diuretic. Thiazide diuretics act in the kidney to remove water from the blood into the urine, by causing an increase in the removal of salts such as potassium and sodium from the blood. This removal of salts causes water to be drawn out of the blood and into the kidneys, where it is then excreted in the urine.

Removing water from the blood decreases the volume of fluid circulating through the blood vessels. The drop in fluid volume results in a drop in the pressure of the fluid within the blood vessels, which helps to lower high blood pressure. It also decreases the effort required by the heart to pump blood around the body.

In this way Generic Vaseretic controls blood pressure.

Food and Drug Interactions with Vaseretic

  • Non-steroidal anti-inflammatory drugs (NSAIDs) e.g. indomethacin, may reduce the effect of enalapril on lowering blood pressure.
  • If Generic Vaseretic is taken together with potassium sparing diuretics, potassium supplements or potassium containing salt substitutes there is an increased risk of raised blood potassium level (hyperkalaemia). These should only be used with frequent monitoring of blood potassium levels.
  • When Generic Vaseretic is taken together with medicines that decrease blood pressure there is an increased risk of a large drop in blood pressure.
  • Generic Vaseretic reduces the ability of the body to remove the medicine lithium, which may give rise to adverse effects.
  • When Vaseretic is taken together with other medicines which can cause low blood potassium level (hypokalaemia) e.g. corticosteroids, carbenoxolone or beta-agonist bronchodilators such as salbutamol, there may be an increased risk of developing hypokalaemia.
  • Generic Vaseretic may raise blood glucose levels and therefore reduce the effectiveness of antidiabetic medicines. Dose adjustments of the antidiabetic may be required.

Contraindications with Vaseretic
Vaseretic is contraindicated in the following:

  • Allergy to medicines from the sulphonamide group, e.g. the antibiotic sulphamethoxazole
  • Failure of the kidneys to produce urine (anuria)
  • High blood creatinine level
  • History of swelling of the lips, face or tongue (angioedema) caused by enalapril
  • Kidney failure requiring a certain type of haemodialysis (high-flux membrane)
  • Known sensitivity or allergy to any ingredient
  • Pregnancy: When used during the second and third trimesters of pregnancy, enalapril can cause injury and even death to the developing fetus. It is not known whether enalapril passes into breast milk. Do not take Vaseretic without first talking to your doctor if you are breast-feeding a baby.

  • Decreased kidney function
  • Diabetes
  • Disease of the blood vessels in and around the brain (cerebrovascular disease)
  • Gout
  • Heart disease caused by inadequate blood flow to the heart (ischaemic heart disease)
  • High blood uric acid level (hyperuricaemia)
  • Liver disease
  • Long-term inflammation of skin and some internal organs (systemic lupus erythematosus)
  • Narrowing of the artery which supplies blood to the kidneys (renal artery stenosis)
